The practice currently has limited daytime openings for new weekly therapy patients (ages 5-55). After school visit times are currently full. Click here to complete a Consultation Request Form.

The medication management (ages 5-25) portion of the practice is currently full. Openings possible in February. You are welcome to complete a consultation request form now and it will remain on file for six weeks or to check back in February.
